The programs affected are: Horizons Health Care & Retirement Community with a new main number of 970-835-2600, Senior CommUnity Care PACE (Program of All-Inclusive Care for the Elderly) with a new main number of 970-835-2900, and Senior CommUnity Meals with a new main number of 970-835-2670.
The previous main numbers will forward for a short period before being discontinued. The executive directors of each program ask that the public makes a note of the new numbers. The Volunteers of America network of programs greatly appreciates the support from the communities that it serves and strives to make communication technology a top priority to meet the needs of our patients, residents and participants.
