REQUEST FOR QUALIFICATIONS DELTA COUNTY MEMORIAL HOSPITAL DISTRICT DESIGN-BUILD CONSTRUCTION & REMODEL PROJECT The Delta County Memorial Hospital District (DCMH) seeks to prequalify general construction contractors to provide design-build construction and construction management services for the remodel and construction of approximately 12,000 square-feet of medical office and medical treatment space (the Project). Contractors interested in being considered to construct this project must submit one (1) original and two (2) copies of the following: 1. Cover Letter (one page maximum) 2. Description of Project Team, including an organizational chart and resumes for key personnel 3. Documentation of bonding capacity (one page maximum) 4. Documentation of similar medical/health care facility construction and remodel experience 5. Documentation of experience with State codes and permitting 6. Proposed construction schedule 7. List of References (minimum of five required) Submissions shall be made no later than 4:00 p.m. on November 25, 2020: Delta County Memorial Hospital District Attn: George Baier 1501 E. 3rd Street Delta, CO 81416 It is the responsibility of the contractor to hand deliver their submittal by the due date and time or allow sufficient time for the submittal through the U.S. Postage Service. Submittals will not be accepted by email. Late submittals will not be evaluated. Prequalification will be made to the contractors whose responsive qualifications are determined to best meet the evaluation criteria of the Delta Memorial Hospital District. Contractors who meet the qualification requirements will be notified by DCMH and will then become eligible to participate in the bidding process. Only those contractors who are considered by DCMH to be qualified to construct the Project will be invited to bid on the Project. Published in the Delta County Independent November 11 and 18, 2020.
