Delta County Memorial Hospital (DCMH) has joined Monument Health’s network, effective Jan. 1, according to a joined press release from DCMH and Monumental Health.
“Monument Health is a clinically-integrated network (CIN), a group of hospitals, doctors and other health professionals who work together to improve care and reduce costs,” the press release states. “Monument Health is the largest CIN in Western Colorado and was founded on the principles of better care, lower costs and healthier communities.”
Any hospital or provider that joins Monument Health must reduce its prices, be accountable for patient outcomes and coordinate with other providers in the network about patient needs.
DCMH has ranked in the top 100 for rural and community hospitals for three consecutive years, according to the statement.
“Monument Health is pleased to have Delta County Memorial Hospital join our network,” Monument Health CEO Stephanie Motter said. “Their commitment to excellent and affordable health care reinforces the high-quality standard of care Monument Health represents.”
By joining the Monument Health network, DCMH is joining “St. Mary’s Regional Medical Center, Colorado Canyons Hospital and Medical Center, more than 150 primary care providers, and over 450 specialists in the Monument Health network.”
According to the press release, access to the Monument Health network is provided through Rocky Mountain Health Plans (RMHP) insurance plans or through Connect for Health Colorado, the state’s health insurance marketplace.
